Job Description: This is a full-time LPN case management position. The incumbent is responsible for conducting assessments to determine eligibility for personal care services and completing monthly home visits.
Monday-Friday 8:30-4:30PM, No Weekends, No Holidays.
Requirements:
- Licensed Practical Nurse in Massachusetts
- No experience required
-Must have a valid driver's license.
