As a skilled Registered Nurse, you will play a crucial part in ensuring seamless care coordination for patients along the healthcare continuum. Your strong clinical assessment skills and knowledge of medical standards of care will enable you to navigate complex patient needs with ease.
Key Responsibilities:
- Coordinate patient care across various settings
- Identify opportunities for social work interventions and refer cases as needed
- Initiate referrals for home healthcare, hospice, DME, and pharmacy services when appropriate
- Facilitate transfers to other facilities when required
- Collaborate closely with the healthcare team to ensure patient needs are met and plans are implemented effectively
Requirements:
- A minimum of 2 years of experience in Med/Surg, Long Term Acute Care, or a similar field is essential
- Prior experience in Homecare, Emergency Department, or ICU settings is desirable
- A BSN degree and Case Manager Certification Training are mandatory
- Strong analytical, data management, and PC skills are required
