Job Summary
This is a unique opportunity to work as a Social Work Social Worker in Brighton, Massachusetts.
Key Responsibilities
- Perform clinical social work assessments and complete MDS entries within established timeframes.
- Develop case management/social work plans collaboratively with the interdisciplinary team, patient, and family to maximize health care responses and quality outcomes.
- Provide patient education and support to ensure a smooth transition from acute settings to discharge.
- Culturally competent care is displayed by identifying patients' cultural needs and incorporating them into the discharge plan.
Requirements
- Licensure: Current Massachusetts license as LCSW/LICSW.
- Skills:
- Knowledge of community services and healthcare systems.
- Computer proficiency, particularly with Microsoft office applications.
- Highly developed communications and interpersonal skills for working with diverse populations.
- Ability to work independently, prioritize tasks, meet deadlines, and adapt to changing situations.
