Licensed Practical Nurse - 40 hour Day - BWH Urology Clinic
Job Summary
- Review and reconcile physicians' orders and update medical data.
- Communicate pertinent information to involved parties according to system guidelines.
- Primary contact will be with clinicians, physician practices, acute care facilities, skilled nursing, extended care facilities, medical supply companies as well as outside agencies for both current and potential patients.
- Ensure that physician orders and all Medicare-required documentation are received and communicated to the appropriate staff.
- Maintain professional and collaborative internal and external relations with contacts.
- Ensure that clinical and non-clinical data is updated and corrected in the electronic medical record (EPIC) as appropriate.
- Generate inter-agency discipline referrals based on the clinician’s request.
- Assists with patient-related issues resulting in calls to doctor’s offices as well as other health care providers as directed by case managers.
- Assists and may coordinate follow-up with various labs and obtain clinical orders per physician’s request.
- Ensure that clinical documentation and medical orders are entered in accordance with agency standards.
- Coordinate and/or assist with ordering medical supplies and DME and ensure proper follow-up when needed.
- Assist with patient assessments, including vital signs, and report changes in patient condition to the care team.
Qualifications
• Licenses/Certifications: Current LPN Licensure in the Commonwealth of Massachusetts
• Education: Graduate of an accredited Licensed Practical Nursing Program.
• Experience: Some experience in a healthcare setting preferred.

- Strong patient care and clinical skills.
- Proficient in administering medications and treatments.
- Excellent communication and interpersonal skills with patients and healthcare teams.
- Ability to prioritize and manage multiple tasks in a fast-paced environment.
