What are the responsibilities and job description for the ATS Case Manager FULL TIME position at MASSACHUSETTS DETOX CENTERS?
Areas of Responsibility
Complete all clinical documentation for caseload including Biopsychosocial assessment, treatment plans, individual progress notes and collateral contact with supports and aftercare sources.
Developing the patient's discharge/transfer plans to ensure movement to the appropriate levels of care.
Contacting referral sources, providing case consultation and coordination with referral sources.
Coordinating ongoing care for housing and therapeutic needs with the Treatment Team, substance abuse treatment providers, healthcare providers, community resources, and family members.
Participating as a member of a multidisciplinary team for assigned patients.
Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the patient's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.
Case Manager may also perform the work of an Addiction Counselor including:
Obtaining previous records that are relevant to the current treatment episode.
Collaborating with patient to develop a written treatment plan that is patient-centered and recovery oriented and includes goals and measurable objectives.
Providing the substance abuse counseling services specified in the patient's treatment plan.
Reviewing patient throughout the treatment episode according to ASAM, to determine the need for continued services or discharge/transfer.
Reviewing and, where necessary, revising the substance abuse counseling portion of the patient treatment plan to address emerging problems
Completing Biopsychosocial assessments with assigned patients.
Providing active case consultation.
Facilitating groups.
Ability to assess, plan services, and deliver services to individuals in need of substance abuse treatment.
Ability to establish rapport, systematically gather data, determine the readiness for individual treatment and change, and apply accepted criteria for diagnosis of substance use disorders.
Participation in continuing Professional Development.
Knowledge of medication-assisted therapy.
Skilled in meeting patient needs in keeping with state required staff-to-patient ratio.
Ability to provide education and information to individuals and groups, through formal and/or informal processes, concerning their substance abuse.
Ability to identify patient needs that cannot be met by the counselor or agency and assists patients to utilize the support systems and community resources available.
Demonstrated knowledge of accepted principles of patient record management.
Performs other duties as assigned.
Minimum Qualifications
● Minimum Associates Degree or equivalent experience.
● Current and valid Massachusetts Driver’s License.
● Current CPR certification and First Aid.
● Min 1yr experience in SUD/Mental Health Field (or corresponding college coursework) preferred
● 12 Step knowledge and addiction treatment experience preferred
