We are seeking a *Home Care Physical Therapist* to join our *Home Care Team*! This role involves providing physical therapy services to patients in their homes, following a physician-approved Plan of Care. Responsibilities include initial evaluations, planning ongoing therapy, and implementing and revising care plans as needed.
Schedule: Monday to Friday, Day shift, On call, Weekends as needed
Experience: 1 year of Physical Therapy (Required)
