The RAM / WP (Resident Advocacy, Mobility, and Wellness Program) helps residents of JPNDC properties build income, assets, and financial capability through individualized coaching, referrals, and resource navigation. The Sr. Case Manager will support RAM / WP by offering in-depth case management services to clients and preparing them to join the RAM / WP program.ResponsibilitiesReport to the Associate Director on a weekly basisRecruit program participants through various methods, including group presentations, flyering, events, and networkingWork with each participant to complete a case management Individual Training and Services PlanHelp participants set and achieve case management goalsSupport RAMWP Mobility Coaches with client case management needsHelp participants transition into the RAMWP ProgramBuild partnerships with service providers and make referrals as neededEnter notes on interactions with participants and outcomes achieved in a shared databaseReport on program outcomes, keep records of interactions, enter data, clean data, and identify issues affecting program operationsCoordinate community-building events with residentsProvide occasional group workshopsPerform clerical tasks such as photocopying, scanning, filing, faxing, printing, and loggingWork from multiple job sites and meet participants in their homes, if neededWhat It Takes to Be Successful3-5 years of relevant case management experienceAssociate or bachelor's degree in social work or equivalent work experienceExperience working in a diverse environmentExperience with Salesforce, Bonterra, or other CRMsExperience planning and executing community eventsReliable transportationFamiliarity with presenting to groups of peopleProven knowledge of local community resourcesAvailability to work nights and weekendsStrong mediation, verbal, and written communication skillsStrong organizational skills and attention to detailStrong problem-solving and decision-making skillsAbility to work independently and achieve timely completion of assignmentsProficiency in Microsoft Suite, Word, Excel, and PowerPointThis is a full-time role with a flexible schedule, including some nights and weekends, and frequent site visits.Employment Terms / CompensationThe salary for this position is $55,000-$65,000, based on experience.
