SUMMARY
Responsible for providing high quality housing and resource navigation to individuals and families at the Beans and Bread Day Resource Center. Provides case management services for four families living onsite at Frederick Ozanam House Permanent Supportive Housing Program.
PRIMARY DUTIES
Housing Navigation and Assistance through Coordinated Access
- Screens people to determine eligibility for coordinated access housing assistance.
- Meets with eligible clients to complete coordinated access intake.
- Collects relevant documents like homeless verification, disability verification (where applicable) and income verification.
- Enters data into HMIS, Coordinated Access, and Salesforce within 24 business hours.
- Assists clients with completing intake and housing paperwork when matched to a program.
Resource Navigation
- Connects clients to community resources, including but not limited to
- Employment navigation
- Housing
- Health insurance
- Mainstream benefits
- Furniture
- Shelter
- Eviction Prevention
- Mental and Somatic Health
- Assists with completing job and benefits applications as applicable.
- Establishes a network of service providers to link clients to.
- Supports Assistant Director in determining which services and resources will be beneficial to clients when provided on-site.
- Works collaboratively with internal and external stakeholders to ensure appropriate case coordination to ensure optimal communication and transition within the service delivery process.
Case Management
- Provides Case Management Services to four families living at Frederick Ozanam House.
- Completes two face-to-face visits every month, one of which has to be a home visit.
- Completes relevant forms, including but not limited to service plan, monthly budget, and income verification.
- Removes barriers and ensure clients can maintain their housing.
- Provides resource linkage and supportive services as appropriate.
Data
- Enters services and case notes into HMIS systems when clients check in.
- Completes discharge paperwork for people who are successfully matched to housing.
- On a monthly basis, completes discharge paperwork for clients who were auto discharged.
- Collects monthly statistics and submits to Director.
- Assist Program Leadership and Data Quality Team with data requests.
SECONDARY DUTIES
- Keeps updated on most effective means of accessing services available within the larger community and maintains an active database for referrals and community partners.
- Participates in registration and processing of participants via the electronic Homeless Management Information System (HMIS) and Salesforce.
- Participates in monthly training, either in-person or through the Learning Management System.
- Ensures that each participant agrees, understands, signs, and is in compliance with the Participant Service Agreement.
- Provides crisis intervention as and when needed.
- Represent St. Vincent de Paul of Baltimore to various groups, organizations, and service providers appropriately and within organizational standards.
- Upholds the vision, mission and values of St. Vincent de Paul of Baltimore.
- Attends all staff meetings and other meetings as directed.
- Supports a welcoming and positive environment for volunteers and works with them effectively.
- Other duties as assigned within the scope of the position.
Qualifications
QUALIFICATIONS
- Bachelor’s Degree in Human Services, or related field, required and 2 years successful experience in working with persons who are homeless, at-risk for homelessness or those with a low socioeconomic status in an urban environment.
- Knowledge of and respect for confidentiality.
- Having the ability to have empathy to support vulnerable populations.
- Motivational to encourage clients to follow their service plan and engage in service delivery.
- Strong verbal and written communication skills to work collaboratively with internal and external stakeholders.
- Critical thinking and problem solving to determine the best service plan for each client after assessing clients, analyzing notes from health care and social workers.
- Administrative organization to manage several different cases at once.
- Computer literacy to maintain and manage case records.
