MCC’s mission is to improve the health and well-being of pregnant women and parenting families and enhance school readiness for children 0—3. We achieve this through direct service, advocacy, and research, in collaboration with individuals, families, providers, and communities. Join our dedicated team, where you’ll provide culturally sensitive health education and case management services to pregnant and newly parenting families. From home visits to community outreach, you’ll be at the heart of MCC’s mission to strengthen families and communities by providing critical support during life’s most pivotal moments.
What You'll Do
Deliver intensive case management to moderate- and high-risk pregnant women, new parents, and their families through home visits and phone follow-ups.
Support clients in setting and achieving their own health and social service goals.
Connect families to essential community resources while providing impactful health education and disease prevention materials.
Organize and facilitate engaging client meetings, health education sessions, and community health events.
Collaborate with local organizations and attend community health fairs to promote MCC’s mission.
Maintain detailed, confidential case records and utilize MCC’s cutting-edge database to track client progress.

Experience
What We’re Looking For
- Prior experience in case management and community outreach is required.
- Understanding of community health, maternal and child health, and social service resources.
- Strong interpersonal and organizational skills to build meaningful client relationships and manage multiple priorities.
- A commitment to MCC’s mission and values, with sensitivity to the needs of diverse communities.
- A valid driver’s license and willingness to travel in and around Greater Philadelphia for client visits and community outreach.
