Position Summary
Essential Functions
- Provide educational literature, and prevention materials to clients as needed.
- Conduct pre/post surveys to measure impact of community-based groups.
- Assist with community outreach activities to increase enrollment and expand services.
- Recruit program participants interested in group education.
- Utilize evidence-based curriculum to deliver community-based education.
- Support and refer participants needing more intensive follow-up to appropriate agencies.
- Attend community meetings, health fairs and represent MCC as requested by supervisor.
- Maintain group stats and careful records for all community-based groups and follow up with contacts made as necessary for all groups.
- Attend training to advance in knowledge and expertise.
- Meet with supervisor regularly to report on and discuss work
- Adheres to the MCC’s security guidelines and ensures the appropriate handling of sensitive information.
- Ensure confidentiality of client records, sharing pertinent information only with the client’s approval.
- Attends relevant staff meetings to promote communication and execution of goals.
- Attends professional development training sessions and workshops to enhance professional growth.
- Participate in agency projects specific to related initiatives and activities.
- Other duties as assigned within the scope of position expectations.
- Understanding of MCC’s mission, goals, and objectives and ability to work independently with a high level of energy and contribute as part of a larger team.
- Knowledge of the concepts and practices associated with community health issues, particularly with pregnant women, prenatal care, parenting and other services for mothers, young children, and their families.
- Ability to travel in the Greater Philadelphia region and surrounding counties served by MCC.
- Knowledge of and sensitivity to issues facing the community served.
- Knowledge of group facilitation skills.
- Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, community partners, clients, and members of the community.
- Skillful in Microsoft Office Suite and the ability to operate a computer and use a variety of common software programs.
- Ability to organize tasks in an efficient manner to maintain client records.
- Skilled in a variety of multi-media platforms.
- AA degree from an accredited college or university preferred.
- 2 year experience providing direct services in a community setting preferred.
- Lactation certified preferred/willingness to complete certification as part of the training plan.
- Experience with group facilitation preferred.
- Maternal, childbirth, and child health experience preferred.
- Valid drivers’ license
- Available to work evenings and weekends.
- Must be available to travel for training and conferences.
- Bi-lingual preferred (Spanish)
