JOB SUMMARY/ACCOUNTIBILITY:

The Manager of Quality Improvement supervises the programs outpatient ambulatory health service; health insurance premium cost sharing assistance; home and community-based health service programs. This consists of overseeing and providing administrative and programmatic supervision ensuring these programs are successfully meeting their scope of services, work plan objectives, services are being provided within their respective standards of care, reimbursement for services rendered are being processed, data /reports are being submitted timely and within deadline, and all other contractual obligations are being met. Additionally, this position is responsible for all quality improvement activities and plans in accordance with specific service category guidelines for funder directions; agencies quality and compliance requirements; COA accreditation standards, and overall industry best practices. Further, this position conducts patient screenings, intakes, determines eligibility, assists with service coordination, monitors, and ensures service are being rendered within their respective service categories.

POSITION QUALIFICATIONS:

  • MSW or Master’s Degree in related health/human services field, preferred. Experience in program evaluation and work with clients and providers.
  • Comfortable in a culturally diverse environment and demonstrate awareness of the value of cultural competence, sensitivity in task implementation, and comfort engaging in conversations regarding sexual health and well-being.
  • Ability to take ownership and showcase interpersonal skillset. Good verbal and written skills. Able to communicate effectively.
  • Understanding of the functions of patient advocacy and knowledge of rules and regulations concerning HIPAA and client confidentiality.
  • Well-organized and demonstrated ability to prioritize, multiple tasks; self-driven; achieve desired results and adapt favorably to changing priorities. Highly effective problem-solving ability. Analytical ability sufficient to resolve problems requiring the integration of data from diverse sources.
  • Computer literate, proficient in Microsoft Outlook, able to learn and effectively utilize client software data base.

GENERAL RESPONSIBILITIES:

  • Oversees and provides administrative and programmatic over site for outpatient ambulatory health service, health insurance premium cost sharing assistance, and home and community-based health services. This includes ensuring that services are being performed within performance indicators, standards of care, and contractual obligations.
  • Provides supervision over subcontractors providing outpatient ambulatory health service, health insurance premium and cost sharing assistance, and home and community-based health services. This consists of ensuring accurate implementation of service is being done, monitoring services being provided, and utilization goals are being met. Responsible for payment of subcontractors’ bills and tracks their expenditures and budget balance. Monitors service for appropriate medical services/expenditures and tracks specialty referrals/care.
  • Engages in quality assurance and reporting functions consisting of evaluating the program performance of all of Matrix Community Health services and programs. Initiates process improvement activities related to service delivery, program objectives, and grant requirements through policy and procedure development, form/tool development or selection, and training of department staff and contractual providers to promote accurate clinical data input and service delivery. Completes auditing of patient charts. Completes funder reports. Ensure the department remains in compliance with the overall agency’s quality improvement plans as well as successfully maintaining its COA accreditation. Ensures program passes annual funding site visits.
  • Works with clients to obtain feedback regarding all of Matrix Community Health services provided and assessing satisfaction. Initiates, recruits’ clients, and coordinates Matrix Community Health’s community advisory group. Uses the information that was obtained from these functions in order to analyze trends, incorporate the information into quality improvement plan and activities to improve services in order to more appropriately meet client needs.
  • Tracks, initiates, and responds to all of the departments reporting requirements ensuring deadlines are met and thorough results are provided. This includes the management of all of Matrix Community Health data systems ensuring accurate data entry is being completed by deadlines given as well as working with supervisory staff on ensuring corrective action plans are initiated in identifying and in rectifying any deficiencies found. Is the lead in coordinating the programs quality team and in developing the programs annual quality goals and initiatives.
  • Provides direct patient care through conducting intakes and screening with clients to determine eligibility for services provided by Matrix Community Health Outpatient Ambulatory Health Services, Health Insurance Premium Cost Sharing Assistance, and Home & Community Based Health Services and assists with coordination of appropriate services. Gives clients direction and resources, covers staff caseload when they are not available. Provides direct referrals to clients needing immediate assistance. Also confers with other service providers as appropriate.
  • Maintains a relationship with community organizations who are involved in providing services that may be needed by populations served and assists in developing memorandums of agreement with these providers. Maintains awareness of community changes that could impact service delivery and provides feedback on how to deal with them as needed. Explores different outreach venues in being able to reach targeted populations.
  • Assists in identifying new service, funding, program opportunities for the program to engage in. Utilizes epidemiology data in determining how to allocate program resources. Assists in grant writing and in the development of proposals for additional funding / support for services, as needed. Assists with fundraising and promoting the program services.
  • Performs other duties as assigned.

WORKING AND ENVIRONMENTAL CONDITIONS:

Works in multiple different sites including office, medical clinics and community settings including doing patient home visits. Periodically may need to see patient who are hospitalized. It is common to drive to multiple locations, sometimes in inclement weather. Primarily works traditional business hours but at times may need to be able work non-business hours, such as on weekends or evenings.
