Our Job Opportunities
We are seeking experienced Nurse Practitioners to join our team as Clinical Care Specialists. As a Clinical Care Specialist, you will conduct comprehensive health assessments, provide medical history and diagnosis, and develop treatment plans tailored to the individual's needs.
Responsibilities
- Conduct adult/geriatric assessments, including medical history, diagnosis, and treatment planning.
- Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care.
- Develop and implement treatment plans that address the individual's unique needs and circumstances.
- Provide education and support to patients and their families to promote healthy behaviors and self-management.
Requirements
- Masters Degree in Nursing or related field.
- Current RN and NP licensure in state of practice.
- Board certification by AANP, ANCC, or AACCN.
- 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health records.
