Job Summary:
We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Risk Assessment Clinician. This is a PRN/coverage position with no guarantee of hours.
In this role, you will conduct adult/geriatric assessments, medical history reviews, diagnosis, treatment plans, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
