Healthcare Provider - PRN Position

We are seeking experienced nurse practitioners to join our team at Matrix Medical Network.

Key Responsibilities

• Conduct thorough assessments of patients' current health, medical history, medication adherence, social environment, and other risks.
• Develop and implement individualized plans of care, working collaboratively with patients, families, and healthcare teams.
• Provide health education and support to patients and their families, promoting healthy behaviors and disease prevention.

Requirements

To be considered for this role, you must have:

• A Master's Degree in Nursing or a related field.
• Current licensure as a Registered Nurse (RN) and Nurse Practitioner (NP), including prescription authority.
• Board certification in a relevant specialty.
• A minimum of 1 year of experience as a Nurse Practitioner.
