Medical Services Coordinator - Home Visits
Matrix Medical Network is seeking a detail-oriented Medical Services Coordinator to join our team. As a nurse practitioner, you will have the opportunity to work collaboratively with our healthcare teams to deliver high-quality care to our members.
Job Description:
As a Medical Services Coordinator, you will coordinate home visits, skilled facility visits, and telehealth visits to provide comprehensive care to our members. Your responsibilities will include conducting assessments, developing care plans, providing education and support, and documenting results according to Matrix guidelines and protocols.
Key Responsibilities:
- Conduct comprehensive assessments, including adult/geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Collaborate with healthcare teams, including physicians, case managers, social workers, and family members, to ensure seamless care coordination.
- Develop and implement individualized care plans to address the unique needs of each member.
- Provide education and support to members and their families to promote healthy behaviors and self-management skills.
Requirements:
- Must-Haves:
- Master's degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
Travel Requirements:
You will have the ability to travel, a valid state driver's license, and be able to drive a car, with proof of adequate automobile insurance coverage for the state of residence.
