Nurse Practitioner/Physician Assistant
We are seeking a Nurse Practitioner or Physician Assistant to conduct comprehensive health assessments, gather medical history, diagnose and treat patients, provide health education, and coordinate referrals with physicians and case managers.
About the Position:
Our providers conduct in-depth visits that last up to one hour, reviewing a patient's current health, medical history, medication adherence, social environment, and other risks. This provides valuable insight into a patient's overall well-being, helping us improve quality of care and address multiple care gaps with a single visit.
Responsibilities:
- Conduct comprehensive health assessments, gathering medical history, diagnosing and treating patients, providing health education, and coordinating referrals with physicians and case managers
- Work collaboratively with healthcare teams, including physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel as needed
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care
- Provide services in various settings, including home visits and skilled facility visits
Qualifications:
- Master's Degree required or commensurate experience and satisfactory completion of NP licensure
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
- Board certification by a recognized credentialing organization in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant
- Ability to travel, valid driver's license, and adequate automobile insurance coverage
- Strong computer skills and familiarity with electronic medical record (EMR) software
- Excellent verbal and written communication skills with patients, clients, and colleagues
- Positive and outgoing personality, with strong teamwork skills
Travel Requirements:
- Ability to travel, valid state driver's license, and adequate automobile insurance coverage
- Travel required to meet patients in their homes or community environments
