About the Role:
We are seeking a skilled Nurse Practitioner or Physician Assistant to conduct home risk assessments, provide medical history, diagnosis, and treatment, as well as health education and referrals. This role requires strong communication and collaboration skills to work with physicians, case managers, social workers, family members, and other healthcare professionals.
Duties and Responsibilities:
- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up support.
- Perform home visits and skilled facility visits as required.
Requirements:
- National certification as a Nurse Practitioner or Physician Assistant.
- Valid state driver's license and proof of adequate automobile insurance coverage.
- Strong computer skills and familiarity with EMR software.
- Ability to travel in-state or out-of-state and flexibility to adjust to changing situations.
