About the Role:
We are seeking a skilled Nurse Practitioner to join our team on a PRN basis. As a key member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment in patients' homes, providing expert care and support.
Key Responsibilities:
- Conduct comprehensive adult/geriatric assessments, including medical history, diagnosis, and treatment, as well as health education and referrals as needed.
- Collaborate with physicians, case managers, social workers, family members, caregivers, and other medical personnel to ensure seamless patient care.
- Work closely with Primary Care Physicians (PCPs) to provide patient education and follow-up, ensuring optimal patient outcomes.
Requirements:
- A Master's Degree in Nursing or equivalent experience and NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- A current BLS, ACLS, or CPR certification.
- A minimum of 1 year of experience as a Nurse Practitioner.
