The Matrix Medical Network is a reputable provider of clinical services and care management solutions, seeking a Primary Care Provider to join our team. As a Primary Care Provider, you will be responsible for delivering high-quality, patient-centered care to individuals in need of adult and geriatric healthcare services.

About the Position:

We are looking for a dedicated and compassionate Primary Care Provider to provide PRN services to patients in their homes. The successful candidate will be responsible for conducting assessments, developing care plans, and providing education and support to patients and their families.

Responsibilities:

• Conduct comprehensive assessments, gather medical histories, and develop individualized care plans in collaboration with healthcare teams
• Provide education and support to patients and their families, promoting healthy behaviors and self-management skills
• Collaborate with PCPs and other healthcare professionals to ensure seamless care coordination

Qualifications:

• A Master's Degree or equivalent experience and licensure in a relevant field
• Current RN and NP licensure, including prescription authority or the ability to obtain prescriptive authority
• Board certification by a recognized professional organization, such as the AANP, ANCC, or AACCN, in a Matrix-approved specialty
• A current BLS, ACLS, or CPR certification
• A minimum of 1 year of experience as a Nurse Practitioner
