Risk Assessment Nurse - PRN/Home Risk Assessments
Matrix Medical Network is seeking a dedicated Risk Assessment Nurse to join our team. As a nurse practitioner, you will have the opportunity to make a positive impact on the lives of our members.
Job Overview:
As a Risk Assessment Nurse, you will conduct in-depth assessments of our members' health, medical history, and social environment. Your findings will help us better understand the needs of our members and develop targeted interventions to improve their health outcomes.
Key Responsibilities:
- Conduct comprehensive assessments, including adult/geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Collaborate with healthcare teams, including physicians, case managers, social workers, and family members, to ensure seamless care coordination.
- Develop and implement individualized care plans to address the unique needs of each member.
- Provide education and support to members and their families to promote healthy behaviors and self-management skills.
Requirements:
- Must-Haves:
- Master's degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
