JOB SUMMARY :
Primarily responsible for receiving and evaulating referrals for clinical eligibility of Home Health services. Responsbile for entering all required intake information into EMR and communicating information to scheduling and the clinician for patient admission to services.
JOB REQUIREMENTS :
2 Years Medical Surgical Nursing Preferred
2 Years Home Health, Community Health or Discharge Planning Preferred
LPN Preferred
