What are the responsibilities and job description for the Nurse Manager (LPN/RN) position at MaxCare HRS?
We are seeking a Nurse Manager who is passionate about making a difference and improving the quality of life for adolescents and adults within our Residential/ Skilled Nursing Community.
Applicant must be a Licensed Practical Nurse or Registered Nurse in the State of Virginia with two years of experience in healthcare management.
POSITION SUMMARY:
- Responsible for supervisory duties for the home, overseeing licensed practical nurses and direct support professionals (CNAs).
- Work in conjunction with the Program Manager to set work schedules, delegate assignments, assign tasks and evaluate employee job performance, as well as disciplining employees who do not fulfill their job requirements or provide inadequate care to individuals receiving services.
- Responsible for mentoring less experienced nurses, offering clinical advice, set goals and standards for the home, and may hold regular staff meetings in which they give directions or discuss areas for improvement.
- Responsible for monitoring care to ensure it meets the organization’s standards, and review patient records to analyze the effectiveness and efficiency of the care provided within the home.
- Responsible for consulting with the individual’s physician for recommend treatment options.
- Responsible for the individualized review of an individual’s case, especially if it's complicated or if the individual is not responding to treatment.
- Ensure the department is well-stocked with medical supplies, including medications and equipment. In conjunction with the Program Manager responsible for interviewing and hiring Licensed Practical Nurses and coordinating with the RN Nurse Educator for training and staff development needs.
- Responsible for communicating the interests and consulting with senior management if the staff has questions or concerns, recommending changes and improvements, and offering the home’s opinion regarding proposed changes or decisions under consideration by the organization.
- Comply with all rules, regulations, and policies that apply to the organization.
REQUIREMENTS:
- Assists with resident’s schedules and weekly appointments to include coordination of transportation.
- Assist with coordinating training for staff to ensure their certifications are current.
- Act as liaison to all agencies involved in services for residents, i.e. MaxCare and day programs.
- Assists with communication and notification to residents’ families and Authorized Representative.
- Responsible for the management of residents’ medications (e.g. maintain supply, supervise passing of medications, and documentation).
- Participate in required training.
- Responsible for coordinating with the Health Services Director the nursing plan of care CMS 485, physician certification of medical necessity DMAS 62 and nursing part V (individualized services plan)
- Responsible for the daily hygiene and grooming needs of individuals in the home
- Responsible for the maintaining avoidable cause turnover below 20%
