Speech Therapist - Home Health Agency

Maxicare Select, a Legacy Care Partners, Inc company, is seeking a compassionate and dedicated Speech Therapist(s) (ST) to join our Home Health team in St. Lucie County, Florida. We provide Nursing, Therapy, and Aide services to Worker's Compensation Insurance and Private Duty patients. Maxicare Select was established in 2004, is CHAP accredited and has licenses in Miami -Dade, Broward, Palm Beach, Martin, St. Lucie, Indian River, and Okeechobee counties.

Job Summary:

This is a Per-Diem position for an individual who is passionate about providing quality home health care to patients in need. As an ST in our Home Health department, you will utilize  your clinical expertise to assess, teach, and deliver therapy interventions and treatment directed by the physician and established in the plan of care.

Responsibilities:

- Perform initial and ongoing assessments (NO OASIS ASSESSMENTS!) to evaluate patients' health status to determine their specific needs and develop individualized care plans

- Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes

- Provide a written evaluation and plan of care defining speech therapy needs, goals, progress and expected outcomes

- Administer occupational therapy treatment in accordance with physician orders and plan of care 

- Improves or minimizes residual disabilities of the patient

- Returns the individual to optimum and productive level within the patient’s capabilities
- Collaborate with other members of the healthcare team to provide coordinated and holistic care to patients
- Ensure accurate and timely documentation of patient care and progress
- Coordinate and communicate with healthcare providers, families, and caregivers to ensure continuity of care

- Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge
- Adhere to all state and federal regulations, professional standards, and Maxicare Select's policies and procedures
- Maintain confidentiality of patient information

Qualifications:

- Valid and current ST license in the state of Florida
- Minimum of 1 year of home health or related experience, preferred
- Strong clinical and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and prioritize tasks efficiently
- Must be able to pass a background check and drug screening
