What are the responsibilities and job description for the Transition Team Nurse (Facility to Home) Pittsburgh PA position at Maxim Healthcare Services?
Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-120 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the transition team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transitions Team are then phased out of the home and onto the next. Not just anyone can be a transition team nurse. Transition Team Function
Skilled nurses designated by region that are utilized on referrals only with the primary purpose being transitioning patients from hospital to home.
To be deployed on primary referral only
Expectation is that the team is fully replaced/removed from the home after 90 days of service and a 30 day grace period
Transition Team members are signed to a local region.
Based on team member need and location, some TT may support nearby regions when applicable.
Transition Team Requirements
Minimum One Year Of Nursing Experience
Must be approved for TT by internal RN during office interview
Must Pass Office Competency/skills Validation On The Following Items
Trach
Vent
G-Tube
Proper Documentation
Ideal candidate will display above average customer service, communication, and professionalism qualities.
Candidate must display understanding of Maxim s emphasis/requirements of the following items:
Consistent communication with internal staff
Ongoing vigilance with securing PHI, meeting all HIPPA regulations, and security of patient and staff privacy.
Ability to maintain a schedule that will change frequently
Possibility of lengthy commute times to and from work
Fortitude to work in varying home environments
Must be able to work a minimum of 4 weekend shifts a month

Transition Team Pay Scale
- 5 Days PTO to start. Additional 2 per 1800 hours worked. Roll over maximum of 24 hours per year.
- My time also able to be accrued
- Peer Referral bonus for Transition team
- Regular private duty
- $500 for LPN/RN after 80 hours without call off
- $250 for CNA after 80 hours without call off
- Transition Team
- $1500 for LPN/RN after 80 hours without call off
