Patient Care Specialist (LPN/LVN)
About the Role:
We are seeking a licensed practical nurse (LPN) to provide skilled nursing care to Pediatric and/or Adult patients in their own homes.
Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Provide skilled nursing care/services in accordance with prescribed orders on each assignment.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
- Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Requirements:
- Current licensure as an LPN/LVN in the state where you will practice.
- Current TB or Chest X-Ray.
- Current BLS card.
- One year of prior professional nursing experience preferred.
