Job Title : Registered Nurse (RN) - Home Health

Job Specialty : Home Health

Experience : Minimum of 2 years in a home health setting within the last 10 years

License : Current RN Licensure in New York State

Certifications : BLS Certification (Must be obtained before the start date)

Must-Have : Candidates must meet the following requirements

2 years of experience in a home setting within the last 10 years, which may include working in the field with a Certified Home Health Agency or Licensed Home Care Service Agency, or Managed Long-Term Care.
Experience conducting assessments similar to UAS such as OASIS, providing primary care to homebound persons.
Experienced in care management for persons receiving home and community-based services including Hospice and Assisted Living.
Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse caring for a minimum of 2 patients per week.

Job Description :

Perform initial in-home pre-evaluation of patients, including assessment of health status, strengths, care needs, and preferences; guide the development of individualized long-term care service plans.

Review consumer medical documentation and health referral forms, as relevant to each case.

Enter evaluation data into electronic evaluation forms and transmit as required.

Document any concerns, conflicting information, or issues that arise during the evaluation process.

Promote continuity of care to reduce or eliminate fragmentation, duplication, and gaps in treatment plans.

Discuss healthcare options, necessary supports, service vendor options, and waiver options with consumers.

Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan.

Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and service delivery.

Collect quality review data and required documentation to support outcome measurements; record case notes into the CRM.

Identify opportunities for health promotion and illness prevention.

Maintain a comprehensive working knowledge of community resources, payer requirements, and network services for the target population.

Meet all established standards for this position as outlined in the corresponding annual performance criteria.

Perform other duties as assigned by the Regional Nurse Manager or project management.
