SUMMARY OF POSITION :
Responsible to provide direct supervision, in a 1 : 1. Responsible for, but not limited to, adhering to Community Participation guidelines established through MCAR, Inc. and the Office of Developmental Programs (ODP) in providing direct supervision in 1 : 1 community setting. Serve as implementor for individual's goals per annual ISP. Complete computer based charting (Therap), data entry including payroll and attendance, time sheets, and other required documentation. Work is reviewed by Community Participation Program Director and Transitional Coordinator through observation, review of documentation and a written evaluation annually.
ESSENTIAL JOB AND DECISION-MAKING RESPONSIBILITIES :
Direct Care, Administers emergency CPR and first aid utilizing universal precautions as needed; responsible for completing the process for administering prescribed medications to individuals supported across all approved locations within the program; implements ISP goals; completes Community Participation approved activities with the supported individuals; completes daily computer entry of charting and progress notes, assists Individuals with personal care needs; provides instruction, demonstration, hands-on-training as needed, maintains adequate supplies for Individuals to complete available sub-contract and day activities; counts, records, and monitors finished product for quality control; monitors Individuals problems / concerns referring to Transitional Coordinators as needed; responsible for record-keeping, attendance at staff and other meetings, annual ISP reviews; participates in a minimum of 24 hours of annual in-service training, assists in arrival / departure logistics; assists Individuals during lunch and break times; and other duties as may be assigned.
