Job Title: LPN/Resident Care Coordinator
Location: Green Bay, WI
Organization: McCormick Assisted Living, a ministry of St. Paul Elder Services.
Overview:
We are seeking a dedicated and experienced LPN/Resident Care Coordinator to join our nursing leadership team within our assisted living facility in Green Bay. The ideal candidate will have a strong clinical background, excellent leadership skills, and a passion for older adults. As an LPN/Resident Care Coordinator, you will play a crucial role in ensuring the delivery of exceptional resident care while overseeing daily operations.
Key Responsibilities:
- Lead, mentor, and empower the staff, providing guidance, support, and performance evaluations.
- Ensure compliance with all regulatory standards and best practices.
- Develop and implement care plans tailored to individual needs and preferences.
- Foster a positive and collaborative environment for associates, residents, and families.
- Monitor resident outcomes and implement quality improvement initiatives.
- Coordinate staff schedules and manage resources efficiently.
- Provide education and training for staff on new policies and procedures.
Qualifications:
- Current LPN license in the State of Wisconsin.
- A minimum of 3 years of nursing experience, with at least 1 year in a leadership role, is preferred.
- Strong knowledge of geriatric care.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively.
- Ability to be on call and have flexible hours based on the needs of those we serve.
- Ability to maintain positivity during challenging times.
Hours:
- 40 hours per pay period
- Day shift
- On-call rotation
