Position Status: Full-time.
Hours of Operation: The hours will be set by the Director of Nursing/Administrator
Job Summary: The primary purpose of this position is to provide management of resident care as related to the Resident Assessment Instrument (RAI) process in accordance with deferral and state regulations, and as directed by the Director of Nursing and Administrator. The MDS/RAI Coordinator/MDS Nurse is always expected to have access to and follow the most recent version of the RAI User’s Manual.
The MDS Coordinator is accountable for helping achieve the organization’s goals for positive outcomes. This is to be achieved through effective processes, monitoring, training, and implementing systems that will lead to appropriate MDS completion and submission. Care planning, billing practices, utilization of residents’ benefits and accurate reporting of quality measured data.
Essential Functions / Duties:
  • Conduct initially and periodically a comprehensive, accurate, standardized, reproducible assessment of each resident upon admission (not less frequently than every 366 days) and whenever a significant change is identified in the resident’s status as defined in Chapter 2 of the RAI User’s Manual.
  • Assess every resident using the quarterly items set not less frequently than once every 92 days.
  • Conduct PPS assessments, including timely opening and completion, as outlined in Chapters 2 and 6 of the RAI’s user manual.
  • Ensure clinically accurate coding and completion of the MDS assessments.
  • Provide oversight of assessment completion and transmission to the national repository.
  • Review final validation reports and corrections or modifications in response to warnings or errors needed.
  • Work in conjunction with Social Services to oversee the baseline care plan and coordinate the completion of the comprehensive care plan according to regulatory requirements. Ensure the Baseline Care Plan Summary is provided to the resident and/or their representative by the completion of the comprehensive care plan.
  • Work in conjunction with the DON/ADON to assign and coordinate the work of health professionals toward accurate and timely completion of MDS assessments and care plans.
  • Communicate effectively all MDS schedules and schedule changes to the IDT, including but not limited to both verbal and written forms of communication.
  • Be highly involved in determining skilled level of care for Medicare residents and procuring required Medicare-specific documentation; ensure that physician certification of a skilled level of care throughout the Medicare stay is completed; be involved in making eligibility determinations.
  • Ensure compliance with state-specific regulations regarding the RAI process.
  • Obtain Quality Measure, SNF QRP and Five Star reports at least monthly to review with the clinical team. Provide insight and analysis of MDS-based Quality Measures.
  • Serve as a member of the quality assessment and assurance (QAA) and/or the quality assurance and performance improvement (QAPI) committees.
  • Work closely with the admissions coordinator and rehab nurse manager to obtain accurate and complete documentation to support ICD-10-CM diagnosis coding and surgical procedures.
  • Audit and improve staff education/competency as needed to ensure accurate and timely completion of supporting documentation and MDS Assessments.
  • Foster effective working relationship and build consensus.
  • Maintain confidentiality of sensitive information.
  • Plan, organize, prioritize, work independently, and meet deadlines.
  • Use judgment and make sound independent decisions.
  • Work effectively with individuals at all levels of the organization as well as residents, family members, visitors, government personnel, and the public.
  • Be knowledgeable of regulations, practice standards, and procedures as well as laws, regulations, and guidelines pertaining to RAI process.
  • Participate in training, read memos, and belong to associations to stay on top of changes to the RAI and provide education to staff regarding changes affecting them.
  • Possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for ensuring the accurate and timely completion of the RAI documents.
  • Have the ability to learn computer systems, system applications, and related office equipment.
  • Other duties as assigned by either the Director of Nursing or Administrator.
Education and Experiences:
  • Must be a registered nurse (RN), licensed practical nurse (LPN) or licensed vocational nurse (LVN). May be hired in secondary MDS Nurse positions with RN oversight.
  • MDS experience or certification is preferred.

  • Ability to understand, observe, and enforce infection control procedures related to facility services.
  • Ability to observe all facility safety policies and procedures.
  • Ability to understand and recognize potential physical, chemical, and electrical hazards and take immediate corrective action as appropriate.
  • Ability to understand and enforce the Exposure Control Plan as this position risks occupational exposure to blood borne pathogens and other infectious material as an essential function of the job.
  • Ability to safeguard the privacy of Residents’ health information.
  • Ability to speak and read English and follow oral and written directions.
  • Ability to establish and maintain effective working relationships with the public, residents, and staff.
  • Ability to use computers; the ability to use the designated facility computer system (or systems) at a proficient level.
  • Ability to perform other related duties as directed by administration.
  • Must attend staff meetings, in-service classes, and committee meetings as assigned or required.
Physical and Other Requirements: Primarily this position operates in a professional office environment and routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. In addition, this position interacts with residents, family members, staff, etc., under all conditions and circumstances and must be constantly alert for resident safety. This position is also subject to exposure to infectious diseases, conditions, etc., including the Hepatitis B viruses.
This position requires the following abilities as essential to for the position:
  • Ability to sit, stand, bend, stoop, stretch, crawl, crouch, kneel, balance, finger, grasp, push, pull, reach, or walk.
  • Ability to lift objects from a lower to higher position or moving objects along a horizontal level, but from position to position. This includes lifting and carrying weights up to 25 pounds.
  • Ability to exert force upon an object in order to draw, drag, or tug objects toward the source of the force in a sustained motion.
