The Principal Care Manager serves as an integral member of the patients care team, managing the healthcare of patients with a single, high-risk chronic condition, focusing on creating detailed care plans, coordinating with other providers, educating patients about their condition, and monitoring their progress through regular communication and follow-ups, all aimed at optimizing their health outcomes with a specific disease focus; acting as a dedicated care coordinator for complex chronic conditions like diabetes, heart failure, or cancer, where the primary responsibility is to manage that one specific condition comprehensive

SUPERVISION RECEIVED : Reports directly to the Population Health Director

SUPERVISION EXERCISED : None.

PHYSICAL DEMANDS : Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. It requires standing and sitting for extended periods of time. Requires corrected vision and hearing to normal range.

WORKING CONDITIONS : Working in a clinic setting there is potential to be exposed to communicable diseases or body fluids. Work is performed with occasional to frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.

Roles and responsibilities of Principal Care Manager.

  • The Principal Care Manager provides patient-centered care management services to patients and families.
  • This position serves as an integral member of the interdisciplinary care team and will be responsible for identifying care coordination needs in collaboration with the care teams, patients, and patients’ external support system.
  • Within the context of an interdisciplinary team, the Principal Care Manager will participate in progressing patients towards the discharge / next level of care by identifying barriers and implementing appropriate solutions / interventions.
  • The Principal Care Manager will consistently and thoughtfully apply nursing values and continuous quality improvement in daily work.
  • The Principal Care Manager provides patient-centered care management services to patients and families.
  • The Principal Care Manager works within an care team to facilitate the patient’s plan-of-care throughout the continuum of care by ensuring appropriate utilization management, care coordination, resource utilization, and clinical documentation.
  • The Principal Care Manager will function within the McFarland Clinic Clinical Nursing Professional Practice Model, which includes accountability for assessing, planning, implementing, evaluating, and communicating the patient care plan progression.
  • The Principal Care Manager advocates for the patient and alert the rest of the care team to any changes in patient condition.
  • The Principal Care Manager bridges communication between providers, patients and families, members of the care team, and outside agencies to ensure high-quality care that meets the patient's needs and is delivered in a cost-effective and timely manner.
  • Acts as a liaison between patients, health care providers, and health insurers to ensure patients receive excellent health care.
  • Ensures documentation is timely and accurate within the patients record.
  • Considers the patient's unique care needs and advocate for personalized treatment options.
  • Exceptional communication skills, organizational skills, and analytical and problem-solving skills are key to the success of a PCM team member.
  • Potential for Remote work after a 90-day period is completed and approval from the Director is given.
  • Team oriented mind set in Population Health.

Days : Monday - Friday

Hours : 8 : 00 AM - 5 : 00 PM

Education :

  • Graduation from an accredited program for Medical Assistants
  • Graduate of an accredited school of nursing.
  • Certificate / License :

  • Certification or registration from an approved certifying organization for Medical Assistants (AAMA, ARMA, AMT, NAHP, NCCT, NHA).
  • Possession of a State Practical Nurse license.
  • Current State of Iowa Registered Nurses license
  • Current Basic Life Support certification for Health Care Providers.
  • Mandatory Reporting of Dependent Adult and Child Abuse.
  • Experience :

  • Prior experience in the following areas is strongly preferred : utilization management, case management, home care setup, discharge planning, and / or gerontology.
  • The ANA Nursing : Scope and Standards of Practice and Code of Ethics provide a basis for the practice of the PCM Care Manager.
  • Must reside within McFarland Clinic Service areas.
