LPN

As an LPN You will provide support to medically complex adults who have been diagnosed with Intellectual and/or Developmental Disabilities or Autism where they live, work, or in other community settings, to promote their emotional, physical, and personal well-being, and keep patients safe from potential health hazards in their surrounding environment. 

There are  Part-time and Full-time positions available.  Every other weekend &Holiday availability is required.

Pay rate- $30-$32 depending on experience, background and certifications

 In this role, you will:

•         Work with the individual and their team on making and attending medical appointments

•         Transport the individual to medical and other appointments as necessary

•         Provide feedback and observations to the individual’s team

•         Attend team meetings as necessary

•         Assists individuals with activities of daily living in the absence of a DSP

•         Provide guidance in determining the need for emergent care

•         Follow the medical policies set forth by Aegis in conjunction with ODP

•         Always interact with the individual’s team, medical team, and ancillary departments in a professional and respectful manner

•         Support the individuals' preferences and ensure their interests are honored.

•         Help individuals reach their goals to be more independent by offering prompting guidance, instruction/education, and role modeling.

•         Using creative thinking for accommodations to help people with disabilities be more independent

•         Attention to detail, especially when adhering to specific protocols and rules while carefully following instructions to care for the needs of the client. 

•         Complete doctor report forms

•         Observe and collect data regarding human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being, and executing medical regimens as prescribed by a licensed healthcare provider or dentist.

•         Make suggestions based on the individual’s health and safety and medical background

•         Responsible for daily body assessments for those required

•         Assist in wound care, measurements, and reporting guidelines

•         Successfully pass the ODP medication administration class and follow ongoing administration guidelines 

•         Assist in checking and managing an individual's medications.  Ensure medication changes and doctors' orders are relayed to the team and followed through on

•         Successfully manage and utilize the individual’s Electronic Health Record

•         Knowledge of G/J tubes.  Able to assess and appropriately manage, administer medication and apply nursing skills for tube failures as necessary

•         Ability to educate and assist individuals with a diabetic diet, insulin administration, and carb counting

•         Proficient in  obtaining weight, vital signs, blood glucose by finger stick or CGM, measuring intake and output, monitoring skin integrity, G tube/J tube care, urinary catheter care, and wound care

•         Interface with hospital personnel to include doctors, nurses, and medical team for daily updates for hospitalized individuals

•         Integrity to ensure clients feel safe and comfortable when tending to personal activities such as helping them bathe, budgeting, Cleaning, driving, and modeling. 

•          Trustworthiness and dependability so clients and their families feel comfortable relying on them

•         Interpersonal skills when working with clients, including compassion and being sensitive to their emotional needs when they are in extreme distress or pain. Having patience and appropriate self-regulation. 

•         Ability to take directions and use communication skills to accurately and professionally document/communicate interactions, concerns, incidents, and completion of tasks/assignments/trainings.

•         Willingness to accompany those you serve in the community and provide support and assistance in interactions while maintaining sensitivity to the civil and human rights and dignity of the individual.

•         Attending necessary monthly and regulatory/required training regarding client care needs and compliance. 

•         Submitting required annual and personal documentation as needed, according to regulations to maintain compliance.

•          Willingness and ability to work in and travel to an identified individual’s, group, of community homes.

•         Other duties as assigned. 

 

Qualifications:

•         Current LPN license 

•         1 year experience working with individuals diagnosed with Intellectual disability and or Autism 

•         Completion of Background checks as outlined per state regulations.

•         Driver’s background check 

•         Valid driver's license and the willingness to transport individuals in personal or Aegis vehicles, including passenger vans.

•         Available to work rotating weekends and holidays as needed
