McGregor PACE (Program of All-inclusive Care for the Elderly) is a community-based service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing Seniors to remain at home.
We are seeking a highly motivated and organized Activities Coordinator to join our team. As the Community Activities Coordinator, you will be responsible for planning, organizing, and executing various activities and events for our participants.
Responsibilities :
- Implements the coordinated, multi-faceted activity program designed by the Activities Director for frail, older adults, including those with mental illness or dementia, vision / hearing, or physical impairments who reside in the community.
- With guidance from the Activities Director, will plan appropriate therapeutic activities and implement individual and group activities that accommodate the needs of the participants.
- Reports participant status changes to the Activities Director and other staff, as relevant
- Performs initial, and periodic participant assessments for those participants who are considered homebound or do not attend the day center on a regular basis.
- Under the guidance of the Activities Director includes completion of initial comprehensive assessments of participants by enrollment date; formal reassessments with reevaluation of the care plans as needed via facility or home visit, assists in the coordination and documentation of 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; initial and on-going assessments of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team via Recreation Coordinator and reflected on the plan of care as needed.
- Communicate with participants and families regarding activity programming, calendar, and special after-hour events.
- Under the guidance of the Activities Director, provides materials for independent leisure pursuits and small group activities within budgetary guidelines.
- Makes recommendations for purchasing activity supplies according to agency policy and procedure.
- May be required to work in the day center to fill in for the dept. if Recreation Coordinator or at the reception desk as needed.
- Ensures compliance with PACE regulations.
- Assists in departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant.
- Provide educational materials and professional presentations to participants, families, and agency staff, as relevant.
- Takes photos and obtains consent of the participants for the medical charting system as well as activity dept. photos as needed.
- Attends participant outings as needed.

Experience in a multidisciplinary care setting is desirable.
Computer literacy, typing skills, and formal presentation / public speaking experience preferred.
