McGregor PACE (Program of All-inclusive Care for the Elderly) is a community-based service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing Seniors to remain at home.
We are seeking a highly motivated and dedicated Clinic LPN to join our team at PACE. As a Clinic LPN, you will be responsible for supporting day-to-day clinic operations and provides direct nursing care to participants under the direction of a registered nurse.
Responsibilities:
- Within the LPN scope of practice, and pharmacology certification parameters, prepares and administers medications to participants, including oral and parenteral delivery, performs wound care to participants as required
- Within Ohio pharmacy law and LPN scope of practice, prepares medications for participant homegoing and use
- Multidisciplinary team support includes completion of initial comprehensive assessments of participants by enrollment date; assists in the coordination and documentation of a 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; initial and ongoing clinical observation with oversight of RN of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team and reflected on the plan of care as needed
- Assists in departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant
Minimum Qualifications:
- Practical Nurse; licensed to practice in the state of Ohio; certified in pharmacology (CLPN)
- Minimum three years of experience with medication administration, required
- Current Ohio driver's license and reliable transportation required
Preferred Qualifications:
- Experience with community health, long term care, home care, or adult day care, preferred
- Previous experience in the care of the elderly and dementia care, desirable
- Previous experience in multidisciplinary care setting, desirable
- CPR certification preferred
