McGregor PACE (Program of All-inclusive Care for the Elderly) is a community-based service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing Seniors to remain at home.
We are seeking a highly motivated and dedicated Adult /Geriatric Nurse Practitioner to join our team at PACE. As an Adult /Geriatric Nurse Practitioner, you will be responsible for working in collaboration with our Interdisciplinary team in a supportive practice environment. The position requires an energetic, risk-tolerant applicant who enjoys working in a team designed to support elders at risk for nursing home placement and help them remain safely in their homes
Work Schedule: Monday - Friday, day shift
Location: Covering our Forest Hill and Emery Offices (East Side)
Responsibilities:
- Works as primary care provider with on-site primary care physician collaboration to deliver comprehensive health services to their panel of participants, including assessment, diagnosis, clinical decision making, treatment, and prescribing.
- Directly manage acute and chronic illness, promote wellness and preventative care, and follow their patients into various outpatient care settings (including skilled and long-term nursing facilities) in collaboration with the Interdisciplinary Team (IDT).
- Interdisciplinary support includes on-site integrated collaboration with nursing, social work, physical/occupational/speech therapy, transportation, dietary services, and psychiatry services.
- Together with the IDT, provides on-going assessments with a focus on physical, mental and behavioral needs to create and implement a plan of comprehensive care.
- Serves as a clinical resource for nursing staff and preceptor for clinical orientation. Opportunities for precepting students (not required).
- Assists in program and participant outcome data collection and analysis; participates in performance improvement initiatives.
- Is responsible for identifying and working within the scope of practice permitted by Ohio and Federal laws and regulations, the professional code of ethics, and professional practice standards
- Responsible for achieving continuing education and other requirements for licensure, certification and prescriptive authority
- Participates in clinical on-call rotation (from home) as requested
Qualifications:
- Master prepared nurse with certification as a Family Nurse Practitioner or Adult/Gerontology Nurse Practitioner holding active, unrestricted license in the state of Ohio, required.
- Minimum of two years direct care experience working with frail, older adults in community, long-term, skilled care or hospital, required
- Minimum of two years advanced practice nursing experience, required
- Previous experience in an interdisciplinary care setting, desirable
- CPR (BLS) certification, required
