McHenry County Public Health department seeks a full time Clinical Patient Navigation Specialist to work in their nursing division.
37.50 hours per week, Monday through Friday.
Job Summary:
A Public Health Nurse is a registered professional nurse who functions independently and exercises critical nursing judgment. This position ensures the provision of quality nursing care and health education in accordance with best nursing practices. The Public Health Nurse will assist with case management, clinical service and navigation functions of the Illinois Breast and Cervical Cancer Program (IBCCP) in McHenry County.
Job Functions:
- Provides nursing assessment and coordination of breast and cervical cancer screenings for women.
- Provides comprehensive follow-up for clients receiving diagnostic testing according to program guidelines.
- Provides general breast and cervical health education to clients and explanation of Pap tests and/or mammogram results as applicable.
- Responsible for accurate and timely entry of screening data for the assigned caseload.
- Plans, facilitates, and conducts public education/outreach as related to breast and cervical cancer health and screenings to meet grant deliverables.
- Participates in outside department committees, coalitions, and task forces to conduct outreach and fulfill program representation that meets grant deliverables.
MINIMUM QUALIFICATIONS:
- Bachelor’s Degree in Nursing (BSN); or an
- Associate Degree in Nursing (ADN) or Nursing Diploma from an accredited School of Nursing by the Illinois Department of Registration and Education
Other Requirements:
All Health Department staff are required to be up to date on the Covid vaccination series as determined by the CDC, including all primary series doses and boosters, as a term and condition of employment with the Department. In addition, proof of all vaccines are mandatory for employment with the Health Dept., including the flu vaccine.
Any offer of employment is conditioned on the successful completion of a background screening.
