Position Summary
The Director of Social Services at McKnight Place Assisted Living and Memory Care is responsible for establishing a comprehensive program designed to meet the social and psychological needs of the residents and their families. The Director of Social Services is also responsible for overseeing the social well-being of residents. This includes meeting and advocating for residents, communicating for families, assisting in care plan meetings, in-servicing staff on Social Services topics, and being an active integrated part of the Management Team.
Essential Functions, Duties, and Responsibilities :
Achieving psychosocial well-being, promoting continual growth and development of each individual's potential independence.
Schedule, contact, and keep minutes of each resident's individual service plan meeting. Minutes should be distributed and any concerns followed up with.
Complete and maintain the psychosocial portion of each resident's Individualized Service Plan.
Ensure residents and their families are aware of Resident Rights, community support programs, and involving them in family meetings.
Provide education with employees and families on Mental Health, Dementia, or related topics.
Complete a Mini-Mental Status Evaluation (MMSE), Geriatric Depression Scale (GDS), and FAST scale on each new resident who is not being seen by a psychiatrist, within 30 days of move-in.
Make referrals to community social service agencies, should a resident require a service that cannot be readily provided by the facility.
Be an active integrated part of the Management Team and participate in the Manager on Duty (MOD) weekend rotation schedule.
Qualifications :
Education : Bachelor's Degree in social work required, Master's Degree in social work preferred.
Experience : One year in Assisted Living, Skilled Nursing, or Hospital setting preferred.
Oral / Written Communication Skills : Ability to interact pleasantly and effectively with staff, residents and families, and possess clear verbal and written communication skills.
