Home Care and Hospice Resource Coordinator

The Agency Resource Coordinator for Home Care and Hospice is responsible for the coordination and support of the field staff in the delivery of client care services as well as support of business office activities.

The right candidate represents the agency in a professional manner as the primary consumer contact. Demonstrates the organization’s values and principles. Develops effective positive relationships and serves as a liaison with both internal and external customers. Demonstrates ability to shift priorities and triage matters in an effective way.

JOB FUNCTIONS: (This list may not include all of the duties assigned)

1. Coordination of Professional Services

  • Assigns and documents new admission visits in electronic medical record.
  • Monitors and documents staff availability (vacations, meetings, etc.).
  • Coordinates weekend, evening and holiday assignments.
  • Checks tasks and emails daily and follows through on requests in a timely manner.
  • May assist with the communication of HHA orientation to appropriate clinicians and track/monitor supervision visits as required.
  • Facilitates staffing ongoing cases under the guidance of the supervisors of each program.
  • Manages staff call-outs in collaboration with appropriate supervisor.

2. Agency Communication Liaison

  • Effectively triages communications between internal and external customers via phone, fax, e-mail or in person. This includes communication to/from phone services outside of office hours.
  • Facilitates communication of issues to appropriate clinical supervisors.
  • Efficiently and effectively manages relationships and communications between clinical services and office staff.
  • Serves as the agency receptionist and will assist visitors to connect with appropriate agency staff.
  • Maintains relationships with various supply vendors.

3. Agency Administrative Support

  • Responsible for ordering office supplies, some medical supplies and PPE and reconciles appropriate invoices.
  • Manages supply closet/storage.
  • Manages equipment tracking, monitoring and quality checks.
  • Maintains any logs regarding staff in-service education.
  • Maintains master manuals.
  • Assists with new employee orientations as appropriate.

4. Accounts Receivable Support

a. Posts private pay payments in accordance with deadlines for timely payment as well as Netsmart payment entry procedures.

b. Mails paper claims in a timely manner each month

c. Prepares bank deposits in a timely manner and enters contract and supply charges into patient accounts.

5. Lifeline Technician

  • Responsible to do installs and disconnects of Lifeline systemin our client’s home. Responds to trouble-shoot technical problems and teaches clients for regular use and emergencies.
  • Maintains equipment and inventory.

6. Home Health Aide Scheduling and Support

  • Responsible for assigning HHA to clients as ordered by primary care nurse.
  • Develops good working relationships with both professional and paraprofessional staff to assure cooperation and teamwork in scheduling and working together to cover the ordered frequency and duration of visits to provide care to the clients and accommodate staff and time requests.
  • Approves or denies time off requests (with input from clinical supervisors) based on available coverage, previous time off requests, holiday commitments, and if necessary, length of service. Grants time off as far in advance as possible. Tracks all time off requests for the purpose of fairness and consistency.
  • Informs clinical supervisors and administrator (when appropriate) in a timely fashion of any performance issues for Home Health Aides related to attendance, time recording issues, tardiness issues, and in-service attendance concerns. Provides appropriate documentation for counseling provided to the Home Health Aides related to these issues.
  • Documents and immediately informs the clinical supervisor of any complaints or concerns received from client/families related to Home Health Aide performance.
  • Collaborates with staff for timecard receipt, inputs data to EMR or payroll system, clarifies need for changes and corrections to ensure the accuracy of timekeeping records and mileage payments.
  • Assists with new employee orientations as appropriate.

Job Requirements:

  • Ability to manage multiple priorities in a calm and efficient manner.
  • Ability to meet constant demands of clinical staff in the delivery of care.
  • Ability to input data in an accurate and timely fashion.
  • Competency in telephone skills and etiquette.
  • Skill in establishing and maintaining effective working relationships with staff and contacts.
  • Competency in organizational skills.
  • Ability to maintain confidentiality of information.
  • Knowledge and competence in computer functions and the use of computers.
  • Knowledge of home care and hospice process.

Education: High School Diploma

Experience: Healthcare or medical insurance experience. Experience in effectively working with people in managing multiple priorities and meeting deadlines in an efficient manner.

Job Type: Full-time

Pay: $19.00 - $26.00 per hour

Expected hours: 37.5 per week
