What are the responsibilities and job description for the Social Worker - Home Care/Hospice position at McLean Affiliates?
McLean Home Care and Hospice is looking for a 24 hour - Licensed Clinical Social Worker (LCSW) to assist with assessing our patients and family’s needs - psychosocial, environmental, and financial. Formulate, implement, and evaluate a plan of care in collaboration with patient, family, and the caregiving team. Proper documentation per regulatory standards. Monitor, observe, and evaluate changes and progress in patient’s condition and environment and collaborate with the caregiving team.
Qualifications:
Must be a Licensed Clinical Social Worker (LCSW) in the state of employment. Must possess one year of social work experience in a health care setting. Medicare home health or hospice experience is preferred.
