The CENTER for Human Services, a leading provider of behavioral health care services, is seeking a Medical Case Manager. (Full-Time)
Qualifications:
- Bachelor’s Degree in Psychology, Social Work or related field
- Credentialed as a MHP (Mental Health Professional) in the State of Illinois required
- Experience in community behavioral health preferred
- Excellent organizational, documentation, and communication skills
Job Duties:
- Complete screenings to determine client eligibility for services
- Complete comprehensive mental health assessments and develop treatment plans for clients receiving psychiatric services
Salary Range: $19.50 - $24.50 per hour
