The Division of Child and Adolescent Psychiatry at McLean Hospital is offering a two-year, paid Post-Baccalaureate Child & Adolescent Clinical (PBacc) Fellowship Program that combines the opportunity of providing direct clinical care to patients while also providing educational enrichment opportunities, thus increasing chance for future career opportunities after the two-year fellowship is complete. Fellows are initially hired as post-bacc fellows 1. In this capacity, the fellow is under the direction of the Program Manager in each unit, and the fellow provides direct patient care and participates in therapeutic milieu within the scope of defined practice. The fellow also participates in professional education enrichment opportunities, including regular training seminars and clinical supervision, community organization or activity through McLean Hospital, and the expectation to complete a scholarly project by the end of the second fellowship year.

Principal Duties And Responsibilities

  • Provides direct patient care through group and individual interactions in accordance with current treatment plan.
  • Clear, concise, and timely documentation; encounter forms and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines.
  • Intervenes in managing complex patient situations.
  • Remains awake and alert at all times while on the job.
  • Works effectively to manage the therapeutic milieu.
  • Participates in leading and co-leading patient groups.
  • Provides and supervises community activities according to needs. This may include transporting patients to and from activities in hospital vehicles.
  • Provides patients with direction and supervision as needed.
  • Follows established program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Supervises patients in taking medication as prescribed.
  • Follows all program and hospital wide policies and procedures.
  • Reports and documents missed doses to appropriate staff.
  • Communicates observations related to medication side effects and desired effects to appropriate medical staff in a timely manner.
  • Supports nursing staff in assisting patient in acquiring skills to successfully self-medicate.
  • Assists patients in their activities of daily living.
  • Identifies patient’s strengths and deficits related to ADL’s and initiates and implements interventions to meet these needs.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Assists patients in maintaining and keeping various appointments.

SPECIFIC DUTIES for those assigned in inpatient units

  • Participates in the maintenance of the unit environment to ensure safe care
  • Identifies and reports house cleaning problems to Unit Secretary/ND
  • Identifies and reports equipment problems to Unit Secretary/ND
  • Participates in the unit’s inventory system, reporting when supplies are low
  • Initiates activities that assist patients in maintaining a clean environment
  • Assists the RN in providing direct patient care
  • Accurate and timely documentation of AFS, progress notes, and observation sheets
  • Establishes therapeutic relationships with patients, maintaining professional boundaries
  • Participates in patient rounds or teams
  • Completes patient care assignments
  • Identifies patient behaviors that contribute to the development of a crisis; utilizes non-verbal and verbal

Techniques to de-escalate crisis situation

  • Intervenes in managing complex patient situations and patient schedules, utilizing department policies and procedures, always promoting the least restrictive alternatives
  • Communicates changes in patient status to RN
  • Follows Standards of Care and protocols for assigned patients
  • Maintains a commitment to professional growth and competency through the post-baccalaureate fellowship program.
  • Participates in the initial weeklong fellowship orientation
  • Completes mandatory monthly group supervision
  • Participates in mandatory monthly didactic seminar
  • Completes mandatory in-service requirements of each program
  • Identifies areas of strengths and areas needing improvement with supervisor.
  • Maintains competency for treating age specific patient populations.
  • Uses supervision effectively to enhance professional growth and development.
  • Provides peer supervision to a first year fellow on unit
  • Provides peer mentorship to 1-2 fellows and provides support during their adjustment to the new role and unit
  • As part of the educational opportunities, post-baccs will assist with the Child and Adolescent Routine Evaluation implemented in our programs. For that, the fellow will administer baseline questionnaires and scales to new patients
  • Participates in and provides service to McLean and/or the community at large via a

partnership with our office of public affairs and the events and activities they

help coordinate.

  • Fellows also have the opportunity to participate in scholarly work with clinicians on their units resulting on a poster that is presented in McLean research day or another conference, or that results in the production or collaboration in a manuscript.
  • At the end of the two years, the fellows will present a project they created or participated in to the rest of the division.

  • Education; Bachelor’s Degree Required
  • Experience; No experience required. One-year patient care or related experience preferred, preferably in a psychiatric setting.
  • License; Driver's license (MA, CT, NH, NY, RI, VT) preferred (varies by program), and ability to satisfactorily meet hospital driving standards

Skills/Abilities/Competencies Required (MUST be realistic, neither overstated nor understated, and related to the essential function of the job).

  • Excellent verbal and written communication skills
  • Ability to work as part of the clinical care treatment team
  • Given the significant level of psychiatric acuity of patients in residential, and inpatient treatment programs, the work of a post-bacc fellow is often highly emotionally stressful. Typical treatment involves helping patients learn to anticipate and manage emotional conflicts, which can result in periods of great distress and reactivity. Post-bacc fellows are required to be participate in the delivery of care, and to be consistently adherent to the care plan which can elicit this distress in patients. It is imperative that fellows can manage their own thoughts, emotions and behavior in the face of patient distress, and that they maintain a professional demeanor at all times, with respect to patients, families and colleagues.
  • All employees must possess basic computer skills to use a variety of electronic or online systems for communication, clinical and administrative purposes.
  • Medication Administration Program Certification - Applicants who are not currently MAP certified will have a specified time period from the date of hire, to successfully complete a McLean sponsored MAP certification program. Successful passing of the Medication Administration Certification is a requirement of the job.
