Reliance Hospice and Palliative Care is seeking a skilled LVN to provide high-quality nursing care to patients in private homes, skilled nursing facilities, board and cares, and assisted living facilities under the supervision of an RN. This is a full-time position with regular hours from 8am to 5pm.
The ideal candidate will have at least one year of experience in hospice or home health and possess strong communication and organizational skills. A current California LVN license, CPR certification, California driver's license, and proof of auto insurance are also required.

Key Responsibilities:
- Providing nursing care to patients in various settings
- Assisting in obtaining patient assessments, following accurate orders, and diagnosing patient needs
- Performing and documenting nursing interventions, including patient education
- Administering medications and treatments
- Communicating effectively with RNs, patients, families, and physicians
