- Transport clients to and from hospitals, clinics, doctor's appointments, workshops,and any other specialized transportation needs as scheduled. Transportation is provided within, as well as out of county.DUTIES : Calling clients the night before pick up to confirm pick up timeFollowing route and pick-up schedule as prescribed by Supervisor, driving agency vehicle, plans routes to pick up clients and transport to the appropriate facilities and returnMay be required to assist clients in / out of vehicle, as well as assistance in / out of homes and clinicsDriver will be required to operate the wheelchair lift vansPosition may include some special needs, providing direct service to clients on routine basisMust document mileage for each client transportedReport any unusual events or observations made relevant to patient's medical or emotional state, eligibility needs for future transportation, requests for rescheduling, social service referral needsReport any incidents or accidents to your supervisorDriver is required to attend all mandatory trainingsDriver must be able to be credentialed and maintain credentialing by all necessary entities required by Transportation Manager and funding sourcesMaintain medical emergency supplies in vehicle as directed; report if supplies are neededMaintain vehicle to meet safety standards; responsible for cleanliness of vehicle, inside and outReport to supervisor of any additional maintenance or service needed for maintaining the vehiclesComply with Federal Privacy Act RegulationMust be CHAMPS approvedActively Participate in agency fundraisingAll other duties as directed by DirectorQUALIFICATIONS : Must have reliable transportation to and from work between the hours of 4 : 30 am and 5 : 30 pm as schedule dictates.Must be available to work Sunday through Saturday and holidays as schedule dictates.Must be physically able to safely push wheelchair bound clients to and from vehicle and facilities, and secure wheelchair with wheelchair securements and seatbelts provided in vehicle.Must be able to attend and pass all required trainings and background checks required by MCOP and funding sources as a condition of employment.Must be able to pass a pre-employment drug screen in accordance with DOT-FTA regulations, and must agree to be part of a random drug screening pool that complies with all DOT-FTA regulations.High School diploma or equivalent required.Ability to read map, following routine instructions and select alternative route on the spot.Valid driver's license, excellent driving record required.Must obtain chauffeur's license.Must be insurable and obtain First Aid / CPR certification.Physically able to assist clients in / out of vehicles, homes, facilities.Must be able to be approved by Champs and DHHS Health Plans.Ability to relate well to those in poverty with limited language, the elderly, as well as persons with physical and / or mental disabilities.Knowledge of Monroe County, Ann Arbor, Detroit, Downriver and Toledo, OH areas preferred.Ability to accurately complete agency forms - i.e. Sign in / out sheets, time sheets, mileage sheets, client daily schedule sheets.
