Position Summary: Oversee and manage all patient referral and intake operations to ensure seamless admissions and compliance with state, federal, and Joint Commission regulations. Collaborate with internal marketing teams, clinical staff, and external referral sources to deliver efficient, patient-centered care while maintaining accurate records and maximizing third-party reimbursement.
Key Responsibilities:
· Direct daily referral and intake processes, ensuring adherence to regulatory and organizational policies.
· Screen and evaluate patient referrals to confirm eligibility for home health services, facilitating a smooth transition to care.
· Establish and maintain positive relationships with referral sources, promptly processing inquiries and communicating admission decisions.
· Verify primary insurance coverage, secure authorizations, and ensure proper documentation for maximum reimbursement.
· Coordinate with home health staff to provide accurate information for initial patient visits and ongoing care.
· Identify opportunities to improve services and workflows, enhancing patient outcomes and operational efficiency.
· Maintain a comprehensive understanding of company contracts and community resources, redirecting patients when appropriate.
· Communicate effectively with all members of the care team to support coordinated patient care.
· Uphold professionalism, patient confidentiality, and a commitment to continuing education through in-service programs and professional development.

Qualifications
Qualifications
· Minimum of one (1) year experience with admissions in Home Health, skilled nursing facility, hospital discharge, or case management required.
· Knowledgeable of medical practices, terminology and procedures, as well as laws, regulations and guidelines that pertain to home health.
· Able to relate positively, favorably and cooperatively with others, including physicians, associates, and personnel of outside agencies and organizations.
· Must be capable of working in a fast-paced environment by projecting a friendly and professional image.
· Demonstrates good written and verbal communication skills. Demonstrates good negotiation and public relations skills.
