We are seeking Full-time or Per Diem Physical Therapists all over the valley to join one of the largest and fastest growing home health companies in Arizona.

We have therapy needs in the Central Phoenix area, West (Glendale), Southwest valley (Goodyear, Avondale, Litchfield Park, Buckeye) and Southeast valley (Mesa, Chandler, Tempe). 

SUMMARY STATEMENT: The Physical Therapist (P.T.) is a qualified professional who directs, supervises, evaluates and provides physical therapy services to clients in the home as prescribed by the attending physician.

Supervises: Home Health Aide as applicable

DUTIES AND RESPONSIBILITIES:

  • Comply with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
  • Assist the Physician in evaluating clients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests and participates in the Physician’s Plan of Care.
  • Develop, document, implement, and re-evaluate specific Plan of Care noting all treatment modalities and frequencies.
  • Plan administrator prescribed physical therapy training programs for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body part.
  • Make arrangement for outpatient services, which cannot be given in the home.
  • Report to the Physician and Director of Clinical Services or designee the client’s reaction to treatment and/or any changes in the client’s condition.
  • Record clinical/progress notes in the client’s clinical record weekly.
  • Instruct clients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.).
  • Instruct the family in the client’s total physical therapy program and evaluate the home environment.
  • Instruct other involved agency personnel in the care of clients receiving physical therapy.
  • Attend appropriate case conferences and in-service presentations.
  • Plan for client discharge and prepare discharge summaries and other instructions.
  • Participate in staff and agency development activities.
  • When appropriate, supervise Home Health Aides.
  • Document all client-related activities in a timely manner.
  • React to change productively and perform other job-related tasks and duties as assigned.

Qualifications


Education and Experience:

  • Graduate of a physical therapy school approved by the Council of Medical Education of the A.M.A in collaboration with the American Physical Therapy Association or its equivalent.
  • Current Licensure by Arizona State Board of Physical Therapy.
  • Preferably at least one (1) year experience as physical therapist.
  • Ability to lift, push, pull light to moderate loads.
  • Read, write and communicate in English.
