Required Experience
- Possess a high school diploma or equivalent.
- Proficient in Microsoft Office Suite.
- Ability to document outreach activity on forms and in an Excel log.
- Proficient in use of electronic devices (phone or tablet) to communicate to central dispatch, receive information and schedule transport appointments.
- Bilingual Spanish/English Required
- Displays compassionate, ethical, and professional demeanor.
- Extensive travel using reliable personal vehicle.
Duties & Responsibilities:
Responsible for engaging with migrant/seasonal farm workers and informing about InterCare services.
- Plan daily camp visits to designated service area.
- Initiate contact and distribute information to farm workers about InterCare programs and resources.
- Utilize maps, patient charts, and migrant camp directives to locate patients.
- Document enabling services on logs, referral forms, and encounter forms.
- Maintain open and frequent communication with clinic staff and other outreach staff.
- Provide referrals in person, over the phone, or any type of communication to outreach nurses, clinic staff, or other community resources.
- Capture and relay pertinent information when making referrals.
- Identify strategies to facilitate a successful referral.
- Communicating effectively medical terminology to migrant/seasonal farm worker.
- Interpret messages and relay information exactly as spoken to the migrant/seasonal farm worker.
- Use alternative solutions to accommodate migrant/seasonal farm worker understanding.
