Meadow Lakes is seeking an Assisted Living Coordinator (RN) to join our team!
Meadow Lakes is a Springpoint Senior Living Community located in East Windsor, NJ. We are in search of an AL Coordinator (RN) Exempt Position, to join our Management team. At Springpoint, our AL Coordinator plans, organizes, develops and directs the overall operation of the Clinical Services Program in accordance with current federal, state, and local standards, guidelines, and regulations that govern our community.
Responsibilities:
- Assume responsibility and accountability for the direct delivery of Assisted Living nursing services in accordance with the Assisted Living Program regulations.
- Direct the daily operations of the Clinic, including emergency response in Independent Living, oversight of the physicians providing service in the Clinic, and coordinating Personal Care Services for Independent Living residents.
- Conduct an assessment/health assessment on all new residents prior to or upon admission.
- Periodically monitor and evaluate each resident’s physical and mental status for significant changes as per regulations and established policies and procedures. Consult with physician and conduct family meetings as needed regarding changes in level of care.
- Respond to all emergencies while onsite at the community; be available to receive communication 24 hours/day in the event of an emergency.
- Ensure that all residents have annual evaluations or physicals with physician certification as required per regulations and established policies and procedures.
- Review and ensure completion of monthly wellness checks on all residents.
- Monthly audits of MARs/TARs.
- Review all incident reports and coordinate care as needed.
- Conduct documentation audits as appropriate.
- Conduct staff education and in-services on an annual and as-needed basis. Conduct department meetings monthly and as needed with individual staff.
Education and Experience:
- Current and valid New Jersey RN license and CPR certification.
- Two (2) years’ experience in assisted living, medical office, or long-term care setting.
- Must have at least two (2) years supervisory experience.

Salary Range: $100,000.00 - $108,000.00 / year
