Social Service Coordinator

Job Summary
Meadowbrook in Bolingbrook is now hiring a Social Service Coordinator to join our team.

The Social Service Coordinator is responsible to appropriately transition the resident and families through the care process.

Responsibilities:

• Manage QIDP's/Case managers in ensuring Care Plans are updated and relayed to Direct Care staff to help residents meet goals.
• Motivate, encourage residents.
• Assess cognitive capacity and mood utilizing standard assessment tools
• Knowledge of how to adapt life skills to the cognitive and functional ability of each resident and attempt to meet goals.
• Implement Healthcare Directives and have knowledge of end-of-life, palliative, and hospice care.
• Have the ability to discuss end-of-life, palliative, and hospice care with residents and families.

Requirements:

• Minimum of BS in Social Work or related Human Services Field.
• Prior experience in Social Service work in a LTC setting preferred.
• Possess written and verbal skills for effective communication.
• Competent in organizational & time management skills.
• Demonstrates good judgment, problem solving and decision-making skills.
• Knowledge of state and federal resident long-term care regulations in a LTC setting.
• Excellent communication, leadership, and mentoring skills
• Must demonstrate strong conflict resolution.
• Proficiency understanding Medicare/Insurance/Medicaid

Pay: From $55,000.00 per year
