Meadowlark is seeking a candidate to fill a full time Home Health LPN position, to administer skilled nursing care to clients in Topeka, and surrounding areas.
This is performed in accordance with the physician orders and plan of care under the direction of the Registered Nurse (RN). Services are furnished in accordance with the Agency policies. The LPN is delegated the administrative authority, responsibility, and accountability necessary for carrying out the LPN assigned duties. The LPN maintains a professional work relationship with the home health team and clients, and remains knowledgeable of clients’ responsibilities and rights, including the right to determine their own preferences.
ESSENTIAL JOB FUNCTIONS
- Travels to clients homes to provide skilled nursing care within licensure, level of competency, and the established plan of care delegated by Registered Nurse.
- Observes the status of clients condition, utilizes skills of observation of physical and psycho-social characteristics and report’s findings to the registered nurse.
- Ensures proper documentation of visit, notes treatment administration, care, vital signs, progress, adverse reactions to medications, pain levels, and concerns, records and reports symptoms and changes in client's condition to the RN and other home health care team to assure safety and continuity of care.
- Reinforces the information, instruction and education provided to the client’s family members and caregivers by the RN.
- Promotes client’s well-being, by providing nursing care or emergent care services; assesses needs, establishes and implements the plan of care, and advocates for client and family preferences.
- Must utilize any and all safety precautions while in clients home and when carrying out care giving duties, maintains a safe, comfortable, and functional home environment by assessing the home for potential hazards and encourages continuous quality improvement.
- Implements practices, utilizes appropriate techniques regarding sanitation, infection control, fire safety, disaster preparedness and emergency care.
- Performs client positioning, client transfers, client ambulation, and range of motion (ROM).
- Assists client with personal care tasks including assisting with bathing, dressing, toileting and grooming including shaving, skin care, hair care, nail care, and oral hygiene needs. Performs catheter, ostomy, and incontinent cares.
- Interacts with clients, family members and others, listens to clients and families questions and concerns and responds in a timely manner, and communicates any concerns to the RN.
- Maintains working relationships with RN, physicians and other medical personnel through clear and effective communication and maintains client confidentially at all times.
- Recognizes early signs of changing conditions, uses all available and appropriate resources, and seeks consultation with RN.
- Performs general housekeeping duties such as making the bed, changing bed linens and cleaning the bath tub and bath area.
- Maintains confidentiality of all client information.
- Personally responsible and held accountable for following Meadowlark Hills’ mission and values and for assuring resident safety at all times.
POSITION REQUIREMENTS
Must have a valid Driver's license and pass a driver record review, and a drug screen.

Experience: Minimum two (2) years clinical nursing experience preferably in a home health agency or any combination of education and experience that provides equivalent knowledge, skills or abilities. The Home Health LPN is expected to have acquired the necessary knowledge and skills to perform the job satisfactorily in accordance with current standards, guidelines, and regulations of Meadowlark Hills.
Licensing Requirements: Licensed in the state of Kansas as a Licensed Practical Nurse (LPN) required. Certification in cardiopulmonary resuscitation (CPR) required. Possess a valid vehicle driver’s license, car insurance, pass a drug screen and must have access to reliable transportation.
