Position Summary:
The Behavioral Health Associate works in conjunction with the outpatient treatment team in developing, coordinating, and disseminating continuing care plans with patients. Maintains up-to-date referral network.
Primary Responsibilities
- Meets with patients to research discharge planning needs as presented by the treatment team.
- Meets one-on-one with patients from admission until discharge at a minimum of one time per week to discuss case management.
- May be assigned to conduct educational lecture or group activity.
- Communicates with Business Development Department and referring provider to coordinate appropriate aftercare resources.
- Under the guidance of the Clinical Director, reviews and researches aftercare resources.
- Supports treatment team and patients in communication with referral and continuing care resources.
- Documents continuing care plans, communications and meetings.
- Prepares and distributes copies of patient schedules weekly as requested.
- Attends treatment team meetings as assigned.
- Bachelor’s Degree in Behavioral Science or related field preferred.
- One year's experience working in a behavioral health care facility preferred.
- Ability to provide excellent service to internal and external customers.
