About Us: Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks and to support families through day services. Annually, we deliver nearly 2 million meals to 20,000 at-risk older adults.
Location: Anaheim, CA
Hours: Monday - Friday, 8:00 AM - 4:30 PM
Salary Range: $68,640 - $73,228 annually (Salary will be determined based on factors such as geographic location, skills, education, and experience)
JOB SUMMARY: Under the supervision of the Social Services Manager, this position will provide organization/operational support for the Social Services Department. This position has responsibility in being a liaison between our clients/agencies and the Social Services department. This position has the responsibility, and will be the lead in, all aspects of case management, HOM operations, and departmental projects to ensure the efficiencies and best service provided to our clients/city partners and agencies supporting our mission.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Leads all training efforts for the Social Services Department, including training of new hires, interns and case managers.
- Over sees and acts as the lead for internship and volunteers rotating through the Social Services department.
- Maintains a case load of at least 40 clients.
- Arranges and coordinates a network of supportive services (formal and informal), consistent with mutually developed care plans.
- Leads and over sees the case conferences meetings.
- Conducts case file auditing.
- Acts as the liaison for other departments and outside agencies in communication and services coordination.
- Manages and over sees departmental projects with the assistance of the case manager assigned to that project.
- Represents the agency in meetings/networking/outreach efforts at the discretion of the manager.
- Practices safe operating procedures at all times and take necessary steps to help ensure a safe work environment for all staff.
- Performs other duties as assigned, relevant to department functions.
EDUCATION AND EXPERIENCE AND QUALIFICATIONS:
- Bachelor’s degree in social work, Human Services, Sociology, Psychology, or other related field of study required.
- Master’s degree in social work, Gerontology, or other social services related field preferred.
- Minimum three years’ experience working with older adults and three years’ experience conducting case management preferred.
- Thorough knowledge of case management principles and techniques and keen assessment and evaluation skills preferred.
- Sensitivity to and awareness of the special needs and resources required to work with older adults preferred.
- Ability to maintain objective, empathetic relationships with all potential clients, without creating dependency required.
- Working knowledge of programs and services available in Orange County for seniors preferred.
- Ability to maintain professional and confidential standards in client business-related activities required.
- Ability to communicate effectively, both verbally and in writing with clients is required.
- Ability to develop cooperative working relationships with community resources, clients, informal support sources, and other employees required.
- Valid California Driver's License and auto insurance as required by state, and ability to provide own transportation (vehicle) is required.
- Eligibility to be employed in the United States required.
- Extensive knowledge of computer applications (Microsoft Word, Excel, PowerPoint) required.
